- आवेदन शुल्क Demand Draft के माध्यम से ही देय होगा।
- DD “District Health Society – 32266834946” के नाम पर देय होगा।
- DD State Bank of India, Mungeli Branch से देय होना अनिवार्य है।
- गलत नाम से बना DD अमान्य होगा और अभ्यर्थी अपात्र माना जाएगा।
- बिना शुल्क या कम शुल्क वाले आवेदन स्वीकार नहीं किए जाएंगे।
- Demand Draft की मूल प्रति आवेदन पत्र के साथ संलग्न करना अनिवार्य है (फोटोकॉपी मान्य नहीं)।
